Moudy leads the pharmacist team at 5 Parks PCN in Bradford, serving 65,000 patients. His pharmacy team are involved in all aspects of patient care, and in particular manage patients from a CVRM perspective, supporting other clinical teams in a holistic approach to patient management.
His particular interests lie in the CVRM space, with particular specialty of managing Hypertension, Lipids, CKD and Diabetes. With years of experience in this space and focus on developing systems for managing patients at scale, he has used his clinical expertise to upskill teams and use technology where appropriate to enhance patient care.
He also runs a secondary care lipid clinic at Airedale hospital, where he predominantly see patients of High and Very High Risk, optimising their lipids, including LDL-C and residual risk as well as screening for Lp(a).
His areas of practice and expertise, knowledge and networks in these areas provide a good platform for innovation and driving change, focusing on improving outcomes for patients and highlighting methods for risk stratification and optimisation.
