Andrew Noble is a GP with an Extended Role in Older People’s Health at City Health Care Partnership CIC (CHCP). His clinical focus is proactive frailty care and the practical delivery of Comprehensive Geriatric Assessment (CGA) across various settings. Andrew also leads digital improvement for the frailty service, CHCP and is the digital rep on the British Geriatric Society (BGS) Healthcare in the Community Committee. He is passionate about right-care-right-place models that reduce avoidable admissions for patients living with frailty and how these models can be delivered in different settings with the MDT using digital tools.
