NEW AWARD!
FUTURE LEADER IN GENERAL PRACTICE
This award seeks to recognise the new shining stars of primary care who are already making an outstanding contribution to transforming general practice for future generations. This contribution can be at practice level, across a group of practices, PCN/ICS or even nationally. We welcome self-submissions and nominations from colleagues. We are looking for entries from those who have shown strong leadership potential through a recent initiative or have been in a leadership role for maximum 1-3 years.

ADVANCING HEALTH EQUITY AWARD
This award seeks to recognise an individual, general practice team or PCN that has delivered an initiative/strategy that has had a measurable impact on addressing health inequalities and unwarranted disparities in health outcomes within the local population. It is open to individuals/organisations working alone or in collaboration with local authorities, community groups and the voluntary sector. We welcome direct applications and nominations from third parties.
CLINICAL IMPROVEMENT AWARD:
PUBLIC HEALTH & PREVENTION
This award seeks to recognise an individual or team who have demonstrated excellence when working to improve healthcare in their local area; raising awareness, encouraging engagement, and ensuring the wellbeing of the local population. We are looking for projects, strategies, and schemes that have been implemented to improve long-term patient care.

WORKFORCE WELLBEING AWARD
This award seeks to recognise successfully implemented initiatives run by individuals, practices or PCNs that have resulted either directly or indirectly in a measurably positive impact on staff wellbeing. We invite direct submissions and nominations from colleagues.
CLINICAL IMPROVEMENT AWARD:
LONG-TERM CONDITIONS
This award seeks to recognise either an individual or team who have demonstrated excellence when working with patients with long-term conditions including, but not limited to diabetes, MSK, CVD and respiratory conditions; helping to improve their lives and the management of their symptoms. Furthermore, we are looking for projects, strategies, and schemes that have been implemented to improve long-term patient care.
CLINICAL IMPROVEMENT AWARD:
MENTAL HEALTH
This award seeks to recognise an individual or team who have demonstrated excellence when it comes to improving mental health services in their area, through an ongoing high-quality standard of care or through an innovative project or scheme.
General Practitioner of the Year
This award seeks to recognise a GP who has made an outstanding contribution in their area to patient care or professional leadership. It is open to all qualified GPs practising in the UK, including partners and sessional GPs. We invite applications from both GPs themselves and nominations from colleagues.
NURSING AWARD
This award seeks to recognise a Practice/Community Nurse or nursing team who have made an outstanding contribution to patient care. We invite applications from nurses themselves and also from colleagues.
tHe pharmacy practice award
This award seeks to recognise individual pharmacists or teams working in a practice/the community/a PCN team who have made an outstanding contribution to patient care in their local area. It is open to all qualified pharmacists practising in the UK. We invite applications from pharmacists themselves and nominations from colleagues.
PRACTICE MANAGER OF THE YEAR
This award seeks to recognise a Practice Manager who has made an outstanding contribution to their surgery’s performance in the last 18 months. We invite applications from both Practice Managers themselves and nominations from colleagues.
PRIMARY CARE NETWORK OF THE YEAR
This category is aimed at primary care networks (PCNs) which have developed in maturity and provided care in their area that has made a positive difference to the lives of their patients. Demonstrating mature relationships both within the network between practices and reaching the stage of being able to successfully integrate and provide services beyond individual general practice level, with other health and social care partners along with the voluntary sector. Entries could also detail innovative partnerships with other service providers both public, such as fire and police, and private. The PCN will be able to demonstrate a benefit to their patient population using data to evidence the impact.
RECEPTIONIST/RECEPTION TEAM OF THE YEAR
This award seeks to recognise a receptionist or reception team who are making an outstanding contribution to their practice and its patients. We invite applications from receptionists themselves and also from colleagues.
EXTENDED PRACTICE TEAM MEMBER OF THE YEAR AWARD
This award seeks to recognise a member of the wider practice team who has had a positive impact on improving patient care, practice efficiency, team morale or clinical workload in their practice or area. The nominee could be a GP Pharmacist, paramedic, physicians associate, HCA, mental health practitioner, social prescribing link worker or any other member of the team who is not covered within one of the other award categories. A successful application will outline how the individual has excelled in their role, whether a newly created position or as a long-term member of the team, introduced or delivered a new initiative and the positive result that has resulted.
GENERAL PRACTICE TEAM OF THE YEAR
This award seeks to recognise a multidisciplinary team that has had a positive impact on improving the patient experience and care given, in their practice and/or their local area. A successful application will outline how the team made these changes for improvement, how they implemented the initiative or project, and the positive impact that has resulted. The ‘Team’ may work together physically (i.e. within a practice or PCN) or be linked in a project at a regional or national level.
SUSTAINABILITY IN PRACTICE AWARD
This award seeks to recognise an individual or team who have sought to embrace sustainability within practice policies or initiatives. A successful application will outline how the individual/team identified a need, researched the best solutions, and implemented an initiative or project. The application also needs to demonstrate the positive impact that has resulted. Entries may relate to one specific project/programme or a wider campaign within the practice to reduce carbon footprint and waste.
DIGITAL INNOVATION IN PRACTICE AWARD
This award seeks to recognise an individual or team who have embraced the use of technology in order to make clinical or procedural improvements. A successful application will outline how the individual/team identified a need, researched the best solutions and implemented the new technology, initiative or project, and the positive impact that has resulted. The individual or team may have been involved in developing/inventing a new solution however this category is restricted to healthcare professionals and is not for suppliers/manufacturers. Technology can be hardware or software. Examples may include AI, telephony, triage, diagnostic tools or online solutions.
