Sarah Holmes is Chief Medical Officer at Marie Curie and a Consultant in Palliative Medicine based at Marie Curie Hospice Bradford. She supports clinical quality and improvement across Marie Curie services, and works with partners including Royal College of General Practice and Royal College of Pharmacy across primary and community care to strengthen palliative and end of life care delivery. In this session she will translate current policy direction and best evidence into practical steps teams can take now—focusing on earlier identification, coordinated care planning, and the role of integrated neighbourhood working in reducing avoidable crisis care.
